Team Forms

These Forms must be filled out as soon as possible.  New prospective players must fill out the Consent to Treat and Waiver of Liability before they participate in Summer Skates.  All skaters will need them filled out before the Team Tryout in July.  We would appreciate all players, new and returning, fill out the Player Information Sheet as soon as possible.
